Information clause regarding the processing of personal data

INFORMATION CLAUSE ON THE PROCESSING OF PERSONAL DATA

According to Art. 13 of Regulation (EU) 2016/679 of the European Parliament and of the Council of April 27, 2016. on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation - GDPR), we inform that:
1. The administrator of your personal data is the Afterschool Activities Center No. 3 in Łódź with its registered office in Łódź (94-043), ul. Olimpijska 9

3. Your personal data is collected and processed in order to fulfill the obligations of the administrator, pursuant to art. 6 sec. 1 lit. a,b,c of the general Regulation of the European Parliament and of the EU Council 2016/679 of April 27, 2016. (GDPR)
4. When obtaining personal data, the administrator provides the data subject with information whether the provision of personal data is a statutory or contractual requirement or a condition for concluding a contract, and whether the data subject is obliged to provide them and what are the possible consequences of not providing data
5. Your personal data is stored and deleted within the time limits indicated in the uniform material list of files and instructions on the organization and scope of operation of company archives or other legal provisions regulating the time of data processing to which the administrator is subject
6. After completion of processing, personal data will not be processed for any purpose other than the original purpose of processing
7. The recipient of your personal data may be:
• persons authorized by the Administrator to process data as part of their official duties
• entities commissioned by the Administrator to perform activities that involve the need to process data (processing entities)
8. The person whose data is processed within the limits set out in the GDPR has the right to request the administrator to access their personal data, rectify, delete or limit processing or object to data processing, as well as transfer data and lodge a complaint to the authority supervising compliance with the provisions protection of personal data, and in the case of voluntary consent to the processing of personal data - the right to withdraw this consent at any time, which does not affect the lawfulness of the processing, which was made on the basis of consent before its withdrawal
